Bjørn Hofmann omtaler i sin leder i Tidsskriftet nr. 22/3013 (1) noen etiske utfordringer knyttet til eventuell godkjenning av «ikke-invasiv prenatal diagnostisk test (NIPD)» til bruk i fosterdiagnostikk. Bakgrunnen er at Universitetssykehuset Nord-Norge har søkt Helsedirektoratet om å få sende prøver til utlandet for analyse av foster-DNA i mors blod, i den hensikt å avdekke trisomi 13, 18 og 21. Dette er tenkt å erstatte dagens prosedyre som består av kombinert ultralyd og blodprøve (KUB-test). Blodprøvedelen av KUB-test består i analyse av to komponenter (PAPP-A og fritt beta-hCG) i mors blod. Fosterets nakkefold måles med ultralyd og sammenholdt med mors alder gir undersøkelsene et samlet risikoestimat for at fosteret har trisomi 13, 18 eller 21. Risikoestimatet er mer nøyaktig enn om man bare skulle beregne risiko basert på mors alder. Vårt laboratorium analyserer blodprøvedelen av KUB-test for hele landet.
Vi er enige i at NIPD reiser mange etiske og medisinskfaglige utfordringer. Desto viktigere er det å være presis i fremstillingen. Hofmann bruker forkortelsen NIPD som på engelsk står for «non-invasive prenatal diagnosis». Forkortelsen NIPT (non-invasive prenatal testing) er en hyppigere brukt betegnelse, nettopp med tanke på det Hofmann selv også skriver: at slike tester ikke er diagnostiske, men at positive resultater bør bekreftes med invasiv diagnostikk, slik anbefalingene er per i dag. Uansett betegnelse så er det snakk om metoder som baserer seg på at man undersøker fosterets DNA i mors blod. En «ikke-invasiv prenatal diagnostisk test» er ikke en metode, men nettopp en test, dvs. anvendelse av metoden for å avdekke en spesifisert medisinsk tilstand (f.eks trisomi 21). Disse begrepene må ikke blandes sammen. En metode kan ha mange anvendelsesområder, som Hofmann selv påpeker. Dette kan være bruksområder som vi ikke er interessert i å benytte. En diagnostisk test derimot, skal avdekke en definert tilstand som har en medisinsk betydning. Bruksområdet må altså vurderes og reguleres nøye for hver enkelt test som man tenker å benytte metoden til. På samme måte som vi ikke bør forby bruk av skalpell fordi den kan brukes til kjønnslemlestelse, bør vi ikke forby NIPT fordi fosterets DNA kan brukes til å påvise andre tilstander. Hoffmann påpeker at det kan være vanskelig å sette grenser for ikke-invasiv prenatal testing, og at tillatelse til å teste for enkelte egenskaper, men ikke andre, krever begrunnelse. Dette er ikke en ny utfordring som oppstår med NIPT. Alle diagnostiske tester må begrunnes ut fra klinisk nytte, ressursbruk og etiske aspekter.
Hofmann skriver at: «flere av de store fagsammenslutningene har anbefalt at man gjør invasive tester i tillegg. Men da reduserer testen ikke risikoen, slik som forespeilt». Underforstått at man da må gjøre invasive tester for å bekrefte et eventuelt positivt testresultat, er uttalelsen direkte misvisende dersom han med «testen» mener test for å avdekke trisomi 13, 18 og 21. Vi antar at han med «risikoen» mener risiko for abort forbundet med invasiv diagnostikk som fostervannsprøve og morkakeprøve. Her må man sammenligne med det som er dagens situasjon med bruk av KUB-test. Gitt alderssammensetningen i den populasjonen som i dag tilbys KUB-test (median alder 38 år (2)), forventer man at 10-15% tester positivt for trisomi 21 med dagens beslutningsgrense (risikoestimat større enn 1:250)(3). De som tester positivt får tilbud om invasiv diagnostikk. De aller fleste av disse fostrene vil være friske, dvs. at resultatet av KUB-testen er falskt positivt. Dagens test har altså svært lav positiv prediktiv verdi. Dersom man i stedet tilbyr NIPT til de samme kvinnene, vil man få langt færre med positivt testresultat, dvs. at behovet for risikabel, invasiv testing vil bli mindre, og antallet friske fostre som skades som følge av diagnostikken vil reduseres. Det er således «uetisk» å avvise NIPT til dette formålet. NIPT til andre formål må vurderes i hvert enkelt tilfelle.
Universitetssykehuset Nord-Norge har søkt om å få bruke NIPT i nøyaktig samme populasjon av gravide kvinner som i dag tilbys KUB-test. Avgjørelsen om at vi i det hele tatt skal lete etter fostre med trisomi ble tatt for ca. 30 år siden. Det er en viktig, men en helt annen diskusjon. Den må vi ikke blande sammen med om vi skal bruke den beste testen til formålet.
